SIGN-ON BONUSES!
- New case managers - general sign-on bonus of $1,500
- Provisionally Certified Case Managers (no need for pre-service
training - able to take cases immediately and work toward full
certification) - $2,000.00
- Fully Certified Case Managers (can assume full caseload - no
need for training) - $3,000.00
- Paid in 2 increments at 3 months and 6 months of employment.
-Purpose & Impact:Services designed for youth (age 6-17) who are
chronically truant/runaway/ungovernable, are court involved or are
likely to enter the petition process and may require more intensive
and lengthy services. These services may be court ordered or
referred through the local Case Staffing Committee. Services to
youth shall be provided during traditional and non-traditional
business hours in communities, homes and/or schools and shall
connect youth and families to a coordinated, comprehensive array of
services that meet their ongoing needs. This position will provide
Intensive Case Management Services to youth and families, act as
Case Staffing Committee Coordinator and act as a liaison to the
Court. - Will act as part of a team with the ICMS
coordinator.Essential Functions:As needed, the program directly
provides or arranges for and coordinates:
- Assessment
- 24-hour crisis intervention
- Counseling services
- Case management services
- Medical and dental services
- Alcohol and other drug education and treatment
- Family Support services
- Coordination with the Courts
- School services
- Tutoring
- Vocation training and job placement
- TransportationICMS will operate from a strength-based
perspective to help youth and families strengthen and manage the
quality of their lives by:
- Advocating on their behalf
- Initiating change agent activities
- Helping to access supports in the community
- Teaching problem solving skills
- Modeling productive behaviorsPhysical Requirements:
- Possess a valid Florida driver's license and be able to use own
vehicle to perform job duties
- Ability to travel locally and out of local area in the
execution of professional duties and for training/conferences.
- Must be able to work long flexible hours, as
needed.Education:Bachelor's degree in social services and other
requirements as indicated by the grant source. Masters
preferredExperience:At least 2 years of experience in Human
Services fieldSkills:
- Good written and verbal communication, time management,
organizational and interpersonal leadership skill.
- Ability to function in a team setting, and professionally serve
a diverse and multi-cultural client population.
- Dependability, discretion and good judgement are
essential.
- Must possess the ability to establish and maintain effective
relationships with co-workers, and the general public.
- Must be able to function in the community without the presence
of direct supervision.
- Must be able to maintain high level of professional
responsibility in reporting activities, conduct and schedule to
supervisor.Principal Accountabilities:
- Reports to Clinical Manager, however the coordinator will have
the role as lead during service provision
- Team player with co-workers, corporate office staff, and
community partners
- Cost effective program operations
- Adherence to CINS/FINS and program policies, procedures, and
management practices
- Organized, complete and accurate program files, case files,
Case Staffing Committee and other required documentation.
