Oncology is coming to an expanded Gulf Coast Medical Center from
Lee Memorial Hospital .

We will be seeking experienced oncology pharmacists to facilitate
the transition and to train staff when this occurs - Please apply

The expanded Oncology unit (72 large, private patient rooms) will
be staffed by Pharmacists 7 days per week, 10 hour shifts per day.
The pharmacists will work between the hours of 8a - 8p and cover
every other weekend.
This is a decentralized position where the candidate will work
directly with Oncology care givers using an interdisciplinary team
based, patient centered approach to delivering the best possible
The addition of Oncology presents us with some of the highest
acuity patients seen in acute care.
The pharmacist is responsible for the preparation, dispensing, and
monitoring of medications used throughout the facility. The
pharmacist is expected to sustain an environment for the safe,
rational, effective, and efficient use of medications prescribed to
patients of all age groups. Pharmacists are responsible for the
direct supervision of technical support personnel. All job duties
shall be performed in accordance with all applicable State and
Federal laws, applicable regulatory agency standards, professional
practice standards, and policies and procedures of the health
MEDICATION THERAPY MANAGEMENT
--- Accurately and efficiently verifies or transcribes medication
orders into patient medication profiles. Assesses all orders for
completeness to ensure that appropriate medications, dosages,
dosage intervals, route of administration and special
instructions/order comments have been accurately transcribed.
--- Effectively utilizes the patient medication profile, pertinent
lab data, or other drug monitoring parameters to screen for drug
allergies, significant drug interactions, contraindications,
therapeutic duplications, and proper dosage with respect to age and
other patient-specific parameter. Maintains and updates accurate
and complete information in the patient medication profile with
relevant patient specific information (allergies, height, weight,
--- Ensures the clinical appropriateness of medication prescribing
and reconciles and resolves any drug-related problems directly with
the prescriber. Responds appropriately to clinical alerts for
allergies, interactions and therapeutic duplications. Recommends
additions or modifications to drug therapy regimens and
corresponding monitoring plans to prescribers Actively participates
in medication reconciliation efforts for oncology and palliative
--- Actively and consistently participates in supporting
departmental clinical service activities which includes:
--> Providing drug information, clinical consultation, and
clarifications of drug therapy to healthcare professionals.
--> Detecting, assessing and concisely reporting adverse drug
reactions, medication errors, and good catches in the incident
--> Consistently documenting relevant clinical consultations
(iVents) and clinical notes in the electronic medical record.
--> Effectively dosing medications per pharmacokinetic and renal
dosing protocols. Monitors and interprets appropriate clinical data
and recommends rational dosing changes when necessary.
--> Upholding the pharmacy formulary, therapeutic substitution
protocols, and other specific drug protocols as required.
--> Attending interdisciplinary patient care rounds, providing
in-service education to patients, healthcare practitioners, or
other community groups and serving as a Pharmacy representative for
--> Facilitating compliance with applicable medication related
--> Participates in Pharmacist to Patient Medication Education
(PPME) activities and documents completion in appropriate section
in medical record
PHARMACY WORKFLOW MANAGEMENT
--- Demonstrates initiative and is proactive in effectively and
efficiently following through on problems and pertinent operational
issues requiring immediate attention/resolution. Does not leave
unresolved problems for co-workers or next shift and facilitates
communication of handoffs of relevant patient care issues. Uses
sound judgment to make effective decisions. Effectively uses time
--- Verifies the accuracy and labeling of all medications that are
prepared, repackaged, compounded, and dispensed from the pharmacy.
Accurately prepares, reconstitutes, dilutes, calculates, and
measures doses of parenteral medications according to USP 797
guidelines and professional standards of aseptic technique.
Instructs staff on the proper/safe handling of chemotherapy
--- Effectively oversees, directs, and prioritizes the activities
of pharmacy technicians and accurately checks all work completed by
support personnel. Effectively supervises operations of the
pharmacy department when supervising personnel are not on duty.
Performs routine clerical duties and pharmacy technician
responsibilities, as staffing needs dictate.
--- Ensures the appropriate storage, accountability, and security
of medications throughout the facility by supervising or conducting
monthly nursing unit inspections or nursing unit rounds. Actively
participates in inventory control and materials management
functions of the department including compassionate medication
program patients. Assists in maintaining adequate levels of
medications and supplies in active dispensing areas, rotating
stock, and reordering of medications/supplies. Receives, issues,
inventories, maintains accurate records, and executes appropriate
controls for the distribution and storage of controlled substances
throughout the facility.
