50 - per milePOSITION RESPONSIBLITIESThe Speech Therapist (ST) is a
professional member of the health care team who provides Speech
Therapy services to patients in compliance with all applicable
practice acts. The Speech Therapist performs duties within
standards of clinical practice as defined by the Department of
Professional Regulation, the American Speech-Language-
Hearing-Association and the policies and procedures of the agency,
and who functions under the direction of a physician and nursing
supervisor in the deliverance of skilled Speech Therapy care to the
client. - Evaluates the client's level of functioning and hearing
recommends mechanisms to enhance the client's hearing ability. -
Provides initial and ongoing comprehensive assessments of the
client's needs, including Outcome and Assessment Information Set
(OASIS) assessments. - Establishes a Speech Therapy treatment plan
in consultation with the physician and revises it with the approval
of the physician as necessary. - Assists in development and
implementation of the interdisciplinary client care plan as it
pertains to Speech Therapy. - Assures that the physician's orders
are appropriate and discusses necessary changes. - Communicates
with other team members and, when appropriate, instructs them in
speech pathology techniques that they may use while working with
the client. - Teaches, supervises, and counsels the family and
client in the total Speech Therapy program and other related
problems of the client at home. - Evaluates the client, his/her
significant other(s) and the home situation to determine what
instruction will be required, what assistance will be available for
the family in caring for the client, and what other agency and
community services will be required. - Makes arrangements for
out-patient services for procedures which cannot be given in the
home. - Records on a timely basis all evaluation data, treatments,
and client's response to therapy intervention. - Supervises the
Home Health Aide every 14 days when Speech Therapy is the only
professional discipline involved in the client's care. - Records
and reports to the physician the client's reaction to treatment and
/or any changes in the client's condition. - Participates in the
development of periodic revision of the physician's plan of
treatment keeping him/her informed of the patient's status and
obtaining additional orders if necessary. - Communicates with the
physician at least every 30 days or whenever changes occur
regarding the client's status and obtains additional orders, if
necessary. - Evaluates the effectiveness of speech pathology
intervention and updates the client care plan as needed. -
Documents in the EMR system same day as the encounter. - Assumes
responsibility for self -development by continually striving to
improve his/her health care knowledge through educational programs,
attendance at workshops and conferences, active participation in
professional and related organization meetings and individual
research and reading. - Confirms, on a weekly basis, the scheduling
of visits with the Director of Clinical Services (DCS) to
coordinate the patient's visit schedule. - Participates in agency
activities, case conferences and meetings when appropriate.
Participates with staff, clients, and physicians in discharge
planning activities and completes a Speech Therapy Discharge
Summary. Designs measurable goals appropriate for the patient and
documents progress towards goals throughout the patient's episode
of care. - Awareness of the Agency's QAPI program. Active
participation as required per policies and procedures related to
the Agency's QAPI program. - Management and follow-up of customer
complaints, following Agency's complaint policy. - Acute awareness
of patient rights and the Agency's responsibility in honoring
patient's rights. - Awareness of patient rights description in
Agency's service agreement and Agency's admission packet. -
Awareness of appropriate reasons for a patient discharge and
Agency's policy on appropriate reasons for discharge. - Awareness
of potential signs of abuse and neglect and the steps required to
report suspected abuse and neglect. - Awareness and ability to
design and identify patient centered and individualized goals with
realistic measurable timeframes. - Awareness of infection
prevention and control techniques, both within their scope of
practice and within the Agency's policy and procedures. - Awareness
and participation in the Agency's infection control program. -
Appropriate bag technique policy acknowledgement and demonstrated
competence when applicable. - Travel to patients' homes, healthcare
facilities and other locations to provide care, treatment and to
fulfill other nursing responsibilities. POSITION REQUIREMENTS -
Graduate of an accredited Speech Therapy program or equivalent
education. - Licensed in the State of Florida as a Speech
Therapist. - Two years of clinical experience as a Speech
Therapist. - Current CPR Certification. - Licensed driver with
automobile that is in good working order and insured in accordance
with the organization's requirements. - Must display good emotional
health and be able to physically tolerate much standing, stooping
and bending; must meet all agency regulations. - Must be
self-directed and able to work with little supervision and have
good observational, judgment and communication skills.Pinnacle Home
