SUMMARY This pivotal position serves as a transitional step towards
becoming a center clinical P&L leader (e.g., Senior Medical
Director [SMD]) within our organization. The current Center Medical
Director (CMD) will utilize their previous leadership experience to
accelerate their understanding and implementation of our clinical
model. The CMD may concurrently engage in PCP Black Belt Training
and Mastering the Medical Director Essentials as part of an
expedited training pathway. We are a value-driven,
outcomes-oriented organization. This role will involve oversight of
various metrics and competencies that are regularly evaluated to
support our clinicians in achieving success and attaining
Partnership status. These metrics include panel size, patient
outcomes (hospital admissions and sick days), CG CAHPS and net
promoter scores (patient experience), HEDIS and clinical gap
closures, as well as measures of medical cost effectiveness. Each
primary care provider (PCP) will have specific goals related to
these metrics and will collaborate with center and market
leadership, along with their care teams, to meet these targets.
Fostering a positive culture within our medical centers is also
crucial, and CMDs are expected to guide PCPs in championing a
culture of love, accountability, and passion. The CMD will be
responsible for guiding the center’s providers towards:
Accountability for outcomes : The CMD will take responsibility for
outcomes, ensuring high-quality clinical care and
cost-effectiveness for each patient in their panel of at least 300
risk-adjusted patients (75% partial panel). They will recognize
their ability to significantly impact patient outcomes by fostering
trusting relationships and encouraging behavior changes. Health
coaching : Rather than merely consulting on illness, the CMD will
act as a health coach, assisting patients in setting both short-
and long-term health goals, collaborating with them to achieve
these goals, and regularly following up to support their health
improvement journey. Simplifying actions : The CMD will prioritize
and simplify tasks to make behavior changes more actionable for
patients and to guide their care teams towards performance goals.
Leadership : The CMD will lead weekly Super Huddle (SH) and
Transforming Care Meeting (TCM) sessions, while also supporting the
Center Manager/Center General Manager. The CMD will gradually take
on responsibilities of the SMD, including direct supervision,
performance management, and training of PCPs in their assigned
center, while maximizing overall clinical value and engaging
physicians to enhance clinical quality, efficiency, outcomes,
cost-effectiveness, and patient satisfaction. This will involve
monthly leadership rounding, initially conducted alongside the
market clinical leader. Over time, the CMD will increasingly manage
daily clinical and business operations to enhance the center's
financial performance in collaboration with the Center
Manager/Center General Manager. The CMD must demonstrate the
ability to work both independently and collaboratively with other
healthcare professionals. They will closely collaborate with center
and market leadership to ensure adherence to guidelines and
participate in risk and quality management programs, clinical
meetings, and other necessary gatherings that promote patient
health and organizational objectives. Training for the CMD will
encompass: PCP Black Belt Training and PCP Essentials Medical
Director and Center Operations Leadership Pathways Program Medical
Economics and Center Financial Performance Sales and Growth The CMD
will comply with departmental goals, performance standards,
regulatory requirements, quality patient care standards, and
organizational policies and procedures. KEY JOB DUTIES AND
RESPONSIBILITIES Independently provide care and oversee outcomes
for patients with acute and chronic conditions, particularly in
older adults. Assume full responsibility for patient care and
outcomes, seeking specialist consultations as necessary while
remaining actively involved in the patient's detailed care. Engage
with hospitalists whenever one of their patients is hospitalized,
regardless of the hospitalist's affiliation. Be responsible for
assessment, diagnosis, treatment, management, education, health
promotion, care coordination, and documentation for patients with
acute and complex chronic health needs. Lead a care team that
includes a care promoter (medical assistant), care facilitator, and
care coordinator for patients who can visit the office. For
patients unable to attend the office—whether in hospitals, skilled
nursing facilities, long-term care, or home-bound—the CMD will
collaborate with the transitional care team and other resources,
including case managers and telehealth options. Facilitate weekly
Super Huddle (SH) and Transforming Care Meeting (TCM) sessions,
while also supporting the Center Manager/Center General Manager.
The CMD will partner with the Center Manager/Center General Manager
to co-lead center stand-ups and ensure exceptional patient care
across their teams. Participate in biweekly meetings with national
directors and attend annual or biannual clinical leadership
conferences. Actively engage in the management of their center and
provide coverage for other providers as needed. Participate in
recruiting patients for the center and additional providers for the
organization. Foster a positive culture that encourages teamwork,
drives excellent results and relationships, and promotes a ‘best
place to work’ environment, as measured by employee engagement
scores. Upon demonstrating consistent primary care performance
trending towards that of a PCP Partner, and maintaining progress in
medical director training, the CMD will gradually take on the
responsibilities of a Senior Medical Director, which may include:
Managing, mentoring, and coaching PCPs in their assigned center to
achieve outstanding clinical outcomes, including observing other
PCPs’ daily huddles. Conducting leadership rounding with PCPs (with
reduced involvement from the market clinical leader). Collaborating
with the Center Manager/Center General Manager to enhance the
center's financial performance and increase membership. Performing
additional duties as assigned and modified at the discretion of
management. EDUCATION AND EXPERIENCE REQUIREMENTS: MD or DO in
Internal Medicine, Family Medicine, Geriatrics, or a related
specialty is required. Must possess or be able to obtain a State
Medical License for the state(s) where they will practice. Board
certification in Internal Medicine, Family Medicine, Geriatrics, or
a related specialty is preferred; Board Eligibility is required.
Once board certified, the PCP must maintain certification through
necessary MOC, CME, and/or retaking board exams as required. A
current DEA number for schedule II-V controlled substances is
required. Basic Life Support (BLS) certification from the American
Heart Association (AHA) or American Red Cross is required within
the first 90 days of employment. SALARY RANGE: $214,700 - $306,714
