experienced Registered Nurse to join our Structural Heart Team as a
Nurse Navigator. This is a rewarding opportunity to support
patients through their cardiac journey while working alongside a
collaborative, multidisciplinary team. About the Role: The Nurse
Navigator serves as a consistent point of contact for patients from
diagnosis through treatment and follow-up. Youll work closely with
Structural Interventional Cardiologists, CT Surgeons, the
Structural Heart Medical Director, and other members of the Lee
Health Heart Institute. The Nurse Navigator will primarily be
located at HealthPark Medical Center in our Shipley Cardiothoracic
Surgery Center. However, future growth of this program may require
travel to additional locations. Key Responsibilities: Coordinate
care throughout the continuumfrom diagnosis to discharge Act as a
liaison among patients, families, care teams, and referring
providers Deliver education and consultation both in person and
over the phone Connect patients with community resources and
support Advocate for patients, removing barriers to timely,
effective care Qualifications: - Bachelors in nursing required,
Masters preferred. - Active RN license in Florida (or eligibility
to obtain) - Minimum of 3 years of cardiac nursing experience -
Excellent communication, critical thinking, and organizational
skills - Experience in structural heart, interventional cardiology,
