Job Description:
Lee Health, a leader in healthcare within Southwest Florida, is
seeking an experienced RN MDS Coordinator to join our dedicated
team. This critical role involves overseeing the coordination and
facilitation of the Minimum Data Set (MDS) for our facilities,
which is essential for compliant, accurate, and effective patient
care planning and reporting. The ideal candidate will bring a
strong clinical background, keen attention to detail, and a passion
for quality care. Conduct comprehensive assessments of patients to
create accurate MDS as per regulatory requirements. Collaborate
with healthcare team members to formulate individualized care
plans. Ensure timely submission of accurate MDS to the federal
database. Train and supervise staff on MDS procedures and updates.
Monitor and evaluate healthcare outcomes based on MDS inputs.
Proficient in MDS 3.0 Knowledge of CMS regulations Strong
assessment and documentation skills Ability to train and lead a
team Exceptional communication and organizational skills Active
Registered Nurse (RN) license Certified MDS Coordinator (RAC-CT)
