Job Description:

1. Successfully works with patients with substance abuse or co-occurring disorders in an effort to effectively link them to needed resources, coordinate such resources as necessary, and monitor the implementation of such. Examples of resources may include, but are not limited to, recovery oriented support systems, psychiatric services, substance abuse therapy and treatment services, housing, financial, vocational, connection to social activities, and other resources as needed to promote independence and enhance daily living. 2. Routinely advocates on behalf of the patient in order to obtain and maintain needed resources; advocates in a manner that promotes patient independence by encouraging and teaching patients to advocate for themselves. 3. Consistently takes a proactive role in finding and developing community-based resources in order to offer patients a wider array of choices with regard to respective needs i.e. identifying housing options, employers, medical and substance abuse/mental health professionals. 4. Works closely with SalusCare Detox, local hospitals, physician groups, internal programs, and other community agencies to collaborate, facilitate and advocate for services as needed for patients. 5. Routinely consults with other treatment providers on a regular basis in order to monitor services and communicate needs of patient. 6. Routinely provides education as needed to substance abuse or those with co-occurring issues and their collateral supports. 7. Promptly and accurately completes required documentation for each person served in accordance with SalusCare policies and procedures. 8. Conduct minimum of monthly contact in order to provide support and conduct ongoing assessment of needs and monitoring of treatment plan needs. 9. Effectively completes all necessary billing related to patient care and medications. 10. Independently manages daily schedule by prioritizing work responsibilities and individual patient needs. 11. Maintains a minimum productivity level designated by the program of 123 hours per month. 12. Effectively coordinates with other programs inside and outside of SalusCare to identify appropriate MAT candidates and coordinate care needs. 13. Actively participates in supervision meetings with primary supervisor by presenting issues and ideas related to implementation of job duties and by receiving feedback related to job performance. 14. Consistently complies with mandates of: CARF, FADAA, 65D-30, Substance Abuse and Mental Health and Saluscare policies and procedures and routinely demonstrates a sound understanding of and commitment to compliance with these standards. 15. As required, will be included in discharge planning, treatment team meetings, conference calls, and other treatment or rehabilitation facilities as needed or mandated by programs. 16. Attend program meetings, department meetings, and other agency meetings as required. Participate in multidisciplinary case staffings as needed. 17. Regularly demonstrates and models attitudes and knowledge consistent with co-occurring best-practice standards. 18. Consistently demonstrates cultural sensitivity and ensures that cultural issues are incorporated throughout care i.e. assessment, service planning, service delivery, transition and discharge services. 19. Will attain competence with Electronic Health Record software and use it effectively within required time frames. 20. Maintains compliance with staff development training requirements. 21. Reliably performs other related duties as assigned by supervisor. Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and (1) one year of full time or equivalent experience working with adults experiencing substance use disorders or co-occurring. Computer skills in Windows operating systems required. Must be able to attain competence with company electronic health record. Follow up with patients for care coordination services Deliver palliative care and related health care services to children and families Oversee care coordination and health coaching for the patients Manage assigned panel of chronic care patients Ensure that patients are receiving appropriate care Work with patients to plan and monitor care Bringing the benefits of coordinated care to patients Facilitate the daily operations of the department Oversee care coordination for the primary care practice's patients Meet member in various health care settings Interact in multiple care settings Communicate essential patient information to home care clinicians initiating patient care Coordinate care plans, discharge planning, and long term care services Receiving quality care from caregivers Contacting patients to schedule transitional care services and home visits Identify progress toward desired care outcomes; intervening to overcome deviations in the expected plan of care; reviewing the care plan with patients in conjunction with the direct care Identify the primary care team involved in the specialty patient care Utilize assessment skills and risk assessment tools to identify patients with actual or potential care needs that would require care coordination Promote adherence to a care plan Connect patient back to primary care physician and primary care coordinator team
