Care Coordinator

Job Description:

A company is looking for a Care Coordinator.
Key Responsibilities:

Oversee a regional POD of providers and assist with patient service management
Facilitate patient internal referrals for therapy and medication management services
Serve as the main point of contact for patient scheduling, insurance changes, grievances, and discharge requests

Required Qualifications:

1-2 years experience in a patient-facing support role
Educational background in behavioral health
Comfort working in a fast-paced, changing environment
Experience or comfortable with crisis management
Bilingual in Spanish/Chinese is a plus but not required
